Your Role:
- Deliver Primary Care: Conduct comprehensive patient assessments, manage chronic conditions, prescribe medications, and develop personalized care plans.
- Preventative & Holistic Care: Address social determinants of health (SDOH) by connecting patients with community resources.
- Collaborate with Experts: Work closely with nurses, behavioral health providers, and community organizations to provide coordinated, patient-centered care.
- Utilize Telehealth & Technology: Provide care through both in-home visits and virtual consultations to enhance patient accessibility.
- Drive Patient Outcomes: Monitor progress, adjust treatment plans, and ensure continuity of care.
What We’re Looking For:
- Licensed Physician Assistant (PA-C) in the state of practice.
- 2+ years of clinical experience in primary care, geriatrics, or chronic disease management preferred.
- Board Certification (NCCPA) required.
- DEA & Controlled Substance Licenses (or eligibility).
- Experience managing a patient panel and working in a team-based care model.
- Telehealth experience a plus.
- Strong interpersonal skills to build lasting patient relationships.
Physical Requirements & Travel:
- Ability to travel within the community for home visits.
- Comfortable working in various weather conditions and climbing stairs for patient access.
- Must have a valid driver’s license and reliable transportation.

Pay: $110,000.00 – $130,000.00 per year
