Are you a Physician Assistant (PA) looking for a rewarding opportunity in patient-centered primary care? In this role, you will provide comprehensive, high-quality care to aging adults in their homes and via telehealth, ensuring they receive personalized, proactive healthcare.

Your Role:

  • Deliver Primary Care: Conduct comprehensive patient assessments, manage chronic conditions, prescribe medications, and develop personalized care plans.
  • Preventative & Holistic Care: Address social determinants of health (SDOH) by connecting patients with community resources.
  • Collaborate with Experts: Work closely with nurses, behavioral health providers, and community organizations to provide coordinated, patient-centered care.
  • Utilize Telehealth & Technology: Provide care through both in-home visits and virtual consultations to enhance patient accessibility.
  • Drive Patient Outcomes: Monitor progress, adjust treatment plans, and ensure continuity of care.

What We’re Looking For:

  • Licensed Physician Assistant (PA-C) in the state of practice.
  • 2+ years of clinical experience in primary care, geriatrics, or chronic disease management preferred.
  • Board Certification (NCCPA) required.
  • DEA & Controlled Substance Licenses (or eligibility).
  • Experience managing a patient panel and working in a team-based care model.
  • Telehealth experience a plus.
  • Strong interpersonal skills to build lasting patient relationships.

Physical Requirements & Travel:

  • Ability to travel within the community for home visits.
  • Comfortable working in various weather conditions and climbing stairs for patient access.
  • Must have a valid driver’s license and reliable transportation.

Job Type: Full-time

Pay: $110,000.00 – $130,000.00 per year
