Job Overview
The Wound Care Nurse Practitioner (NP) provides compassionate advanced clinical care to patients with complex, chronic, or acute wounds. This role involves assessing, diagnosing, planning, and implementing treatment strategies to promote optimal healing, reduce complications, and improve patient outcomes. The Wound Care NP works collaboratively with interdisciplinary teams, including physicians, nurses, and physical therapists, to develop individualized care plans for patients with various wound types such as diabetic ulcers, pressure injuries, surgical wounds, and venous stasis ulcers.
Who we Are – United Wound Care Centers of Illinois Valley is one of 13 locations nationally focused on Treating Chronic Wounds, Diabetic Ulcers, Pressure Wounds, Slow or Non-Healing Wounds, Venous Ulcers, Surgical Wounds, and more, United Wound Care Centers is the next generation in Wound Care. We utilize an advanced portfolio of skin substitutes that support repair, reconstruction, replacement, or supplementation of the patient’s tissue. Aiming to be the alternative to volume Wound Care, we return referral calls within one business day, see our patients within one week of referral, and serve patients with mobility limitations who require in-home care. Our treatments include personalized care with customized product selection, improving quality of life during and after care.
Key Responsibilities
- Patient Assessment & Care:
- Perform comprehensive assessments of patients with wounds, including history taking, physical examinations, and diagnostic tests in both clinical settings as well as in home care.
- Evaluate wound characteristics such as size, depth, exudate, infection, and stage to guide treatment decisions.
- Diagnose and manage conditions related to wound healing, including infections, chronic conditions (e.g., diabetes, vascular insufficiency), and other factors affecting wound healing.
- Treatment Planning & Management:
- Develop, implement, and adjust evidence-based treatment plans for wound care, including dressings, debridement, standard wound therapy and other therapeutic modalities.
- Coordination of Care across multiple medical disciplines and agencies.
- Provide advanced interventions such as advanced skin substitutes in accordance with clinical needs and compliance.
- Patient Education & Counseling:
- Educate patients and families on wound care techniques, prevention strategies, lifestyle modifications, and proper nutrition to promote wound healing.
- Counsel patients on managing comorbidities, such as diabetes, that impact wound healing.
- Collaborative Care:
- Work closely with multidisciplinary teams to ensure comprehensive care for patients with complex wound care needs.
- Communicate with physicians and home health agencies regarding wound progression and potential changes in treatment.
- Assist in coordinating care between various healthcare providers, including physicians, nurses, physical therapists, nutritionists, and social workers.
- Documentation & Compliance:
- Maintain accurate, thorough documentation of patient assessments, care plans, and treatment progress in electronic medical records (EMR).
- Ensure compliance with clinical and regulatory standards, including infection control protocols and best practices for wound care with a strong focus on advanced skin substitutes.
- Quality Improvement & Continuing Education:
- Participate in quality improvement initiatives related to wound care services and outcomes.
- Stay current on new wound care technologies, research, and clinical practices through continuous education, training, and professional development.
Qualifications
- Education & Certification:
- Nursing Degree (RN), Master of Science in Nursing (MSN), Doctorate in Nursing (DNP) from an accredited program with a FNP or APN certification.
- Current and valid Nurse Practitioner (NP) certification and active license in Illinois.
- Familiarity or Experience in Wound Care/ Wound Care Certified (e.g., Wound Care Certified – WCC) or eligibility to obtain within a specified time frame (preferred). Willingness to complete certificate-based training.
- Current Advanced Cardiovascular Life Support (ACLS) certification (preferred).
- DEA Certification
- Experience:
- 1-3 years of clinical experience in home health, wound care or related specialties.
- Experience in performing wound assessments, debridement, and advanced wound care techniques.
- Experience with managing chronic and complicated conditions autonomously with a strong focus on short term and long-term clinical decision making.
- Skills & Abilities:
- Excellent clinical judgment, diagnostic, and decision-making abilities.
- Strong communication and interpersonal skills, with the ability to educate patients and work collaboratively in a team environment.
- Willingness to learn and collaborate.
- Proficient in electronic medical record (EMR) documentation and basic computer skills.
- Ability to manage a diverse patient caseload and prioritize care efficiently.
- Strong knowledge of wound care practices, including modern wound therapies, dressings, infection control procedures, and advanced skin substitutes.
Work Environment
- Work is primarily conducted in either a home care environment or our outpatient clinic.
- Physical demands may include standing for long periods, lifting and positioning patients, and the use of medical equipment for wound assessment and treatment.
