The Mass General Brigham Medical Group is a system-led operating entity formed by Mass General Brigham to deliver high quality, low cost, innovative community-based ambulatory care. The Medical Group provides a wide range of offerings, including primary care, specialty care, behavioral and mental health, and urgent care, both digitally as well as at physical locations in Massachusetts, New Hampshire, and Maine. The group also offers outpatient surgery and endoscopy, imaging, cardiac testing, and infusion. Job Summary We are seeking a highly skilled and dedicated Advanced Practice Provider (NP/PA) to support clinical operations in our Virtual Clinical Support team. The APP will be responsible for providing patient care, focusing on tasks such as virtual consultations, refill requests, care coordination, and supporting clinical teams with non-direct patient interactions. This role plays a critical part in ensuring patients receive continuous, high-quality care while the overall efficiency of healthcare delivery, reducing primary care burden, and enhancing patient outcomes. Key Responsibilities Clinical In basket Management

  • Prescription Refills: Handle routine prescription refill requests, review patient history, and ensure that refills are appropriate, reducing the administrative burden on PCPs.
  • Patient Messages: Respond to patient inquiries or concerns that come through the EHR, addressing issues that do not require the direct involvement of the PCP.
  • Out of Office Coverage: Review incoming lab results, flagging abnormal findings, and initiating follow-up actions as necessary.
  • Virtual Visits
  • Same Day/Next Visit: Perform virtual consultations for non-urgent issues, providing preliminary assessments and management plans, escalating cases to the PCP when needed.
  • Chronic Disease Management: Manage chronic conditions through virtual visits, ensuring patients adhere to their care plans, making adjustments as needed, supporting PCPs in ongoing care.
  • Care Coordination
  • Referral Management: Manage referrals to specialists or other healthcare services, ensuring that patients are directed to the right level of care in a timely manner.
  • Communication Liaison: They act as a liaison between the patient, PCP, and other specialists, ensuring that communication flows smoothly and that care plans are followed.
  • Patient Education: Provide patients with education on their conditions, treatment plans, and self-care strategies during virtual interactions.

Quality & Process Improvement:

  • Innovation: Participates in continuous process improvement, including thoughtful engagement with process consideration for how to adjust and refine process, providing feedback to leadership as appropriate, and willingness to try new ways of accomplishing overall team goals.
  • Quality Assurance: Participate in quality improvement initiatives and ensure compliance with clinical guidelines and organizational policies.
  • Continuous Learning: Stay updated on the latest clinical practices, telehealth trends, and EPIC EHR functionalities to enhance patient care and efficiency.

Job Summary

The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation. Does this position require Patient Care? Yes Essential Functions

  • Provides direct care, counseling, and teaching to a designated patient population in the ambulatory, inpatient, operative, and/or procedural setting
  • Performs complete histories and physical examinations
  • Orders, interprets, and evaluates appropriate laboratory and diagnostic tests
  • Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings
  • Consistently provides high quality and timely documentation including admission and progress notes, procedure notes, operative notes and discharge summaries
  • Performs bedside procedures as are appropriate to the patient population
  • Orders medications and writes prescriptions according to organizational and regulatory policies and procedures

Qualifications

Education Master’s Degree Nursing required Can this role accept experience in lieu of a degree?   • Skilled in taking medical histories to assess medical condition and interpret findings.

  • Ability to react calmly and effectively in emergency situations.
  • Ability to interpret, adapt and apply guidelines and procedures.
  • Ability to communicate clearly and establish/maintain effective working relationships with patients, medical staff and the public.
