Job Summary
We are seeking a compassionate and highly skilled Physician Assistant, Certified (PA-C) to join our multidisciplinary team focused on chronic disease management and delivering high-quality, patient-centered care. The ideal candidate will provide comprehensive care to patients with chronic illnesses such as diabetes, hypertension, heart disease, and other long-term conditions, emphasizing prevention, education, and quality improvement initiatives. This is a bilingual role for Spanish / English servicing an underserved population. Additional languages are a plus!
Key Responsibilities
- Conduct thorough medical histories, physical examinations, and diagnostic evaluations for patients with chronic conditions.
- Develop, implement, and monitor individualized care plans to manage chronic diseases and improve health outcomes.
- Prescribe medications and treatments while monitoring for effectiveness and adherence.
- Provide preventive care and screenings in alignment with evidence-based guidelines.
Chronic Disease Management:
- Collaborate with patients to establish self-management goals and empower them through education and support.
- Coordinate care across multiple specialties and services to ensure continuity and a holistic approach to treatment.
- Utilize electronic health records (EHR) to track patient progress, identify care gaps, and implement timely interventions.
Quality Improvement:
- Participate in quality assurance programs and initiatives to enhance care delivery and patient satisfaction.
- Stay up-to-date on best practices, guidelines, and innovative approaches in chronic disease management.
Patient Education & Advocacy:
- Educate patients and families about chronic disease prevention, lifestyle modifications, and the importance of routine follow-ups.
- Serve as a resource for addressing social determinants of health that impact chronic disease outcomes.
Collaboration & Communication:
- Work closely with physicians, nurses, care coordinators, and other healthcare professionals to deliver team-based care.
- Communicate effectively with patients, families, and caregivers, ensuring they understand care plans and recommendations.
Qualifications
Education:
- National Commission on Certification of Physician Assistants (NCCPA) certification.
- State licensure as a Physician Assistant in [Insert State].
Experience:
- Minimum of 2 years of clinical experience in chronic disease management, primary care, or a related field preferred.
Experience with EHR systems and data-driven quality improvement initiatives.
Skills:
- Strong clinical judgment and diagnostic skills.
- Excellent communication and interpersonal abilities.
- Proficiency in patient education and motivational interviewing techniques.
Work Environment
