Position Overview:

We are seeking a compassionate and dynamic Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join MØDE Direct Primary Care (DPC), a healthcare model centered on innovation, collaboration, and wellness. In DPC, patients pay a flat, recurring membership fee directly to their provider, bypassing traditional insurance billing and focusing on personalized, transparent care.

In this role, you will deliver comprehensive healthcare services, including assessment, diagnosis, treatment, and follow-up care for a diverse patient population. You will have the autonomy to manage your practice while collaborating with a supportive team dedicated to excellence, empathy, and innovation. Your position will be pivotal in supporting and expanding our DPC practice, placing you at the forefront of advancing wellness in our thriving community.

Our DPC model offers patients extended appointment times, enhanced access to their provider through phone, email, or telemedicine, and a focus on routine check-ups, preventive care, chronic disease management, and minor procedures. By reducing administrative burdens, the model strengthens the doctor-patient relationship and emphasizes affordability and comprehensive care.

Responsibilities:

  • Patient Records &Treatment Plans : Maintain accurate and timely documentation in the electronic medical record system. Perform thorough patient assessments, including history taking, physical examinations, and diagnostic evaluations. Develop and implement individualized treatment plans in collaboration with the care team. Prescribe medications and therapies within the scope of practice and organizational guidelines. Educate patients and families about medical conditions, treatments, and preventive care.
  • Quality Improvement: Participate in quality improvement initiatives, contributing innovative ideas to enhance patient outcomes and streamline care delivery. Assist in developing, reviewing, and updating clinical care protocols to reflect current best practices.
  • Care Coordination: Collaborate with multidisciplinary teams, including physicians, nurses, and specialists, to provide comprehensive and coordinated care. Proactively follow up on referrals and ensure timely communication of test results and updates with patients and the care team.
  • Procedure Management: Perform minor in-office procedures such as ear lavage, suturing, wound care, and biopsies, ensuring patient comfort and safety. Assist with or independently perform advanced procedures based on specialty needs and organizational guidelines.
  • Professional Development: Stay current with advancements in medical practices and holistic medical space through continuing education, attending professional conferences, and maintaining certifications. Contribute to mentoring and training initiatives for new staff, students, or peers, fostering a supportive and knowledgeable team environment.
