Physician Assistant – At-Home Patient Care

Job Summary: Optimum Care Partners is seeking a dedicated and compassionate Physician Assistant (PA) to provide high-quality, patient-centered healthcare to individuals in their homes. In this role, the Physician Assistant will conduct in-home assessments, diagnose medical conditions, prescribe treatments, and manage chronic illnesses for patients requiring home healthcare. The ideal candidate will have experience in primary care, geriatrics, or home health and will be committed to delivering personalized care to improve patient health and quality of life.

Key Responsibilities:

  • Patient Care & Clinical Assessment:
  • Conduct comprehensive health assessments for patients in their homes, including physical exams, health histories, and evaluations of medical conditions.
  • Diagnose and manage acute and chronic conditions, developing individualized treatment plans in collaboration with the supervising physician.
  • Order, perform, and interpret diagnostic tests (e.g., lab work, imaging) as needed, and adjust care plans based on test results and patient feedback.
  • Prescribe medications, treatments, and therapies based on patient assessments, adhering to evidence-based practices and clinical guidelines.
  • Provide ongoing care for patients with chronic conditions, ensuring proper disease management, medication adherence, and lifestyle changes.
  • Chronic Disease Management:
  • Manage chronic conditions such as diabetes, hypertension, heart disease, and respiratory disorders by developing and implementing long-term care plans.
  • Monitor and assess patient progress during home visits, modifying treatment plans and medication regimens as needed to optimize health outcomes.
  • Educate patients and families on managing chronic illnesses, promoting preventive care, and making necessary lifestyle changes to improve health.
  • Care Coordination & Collaboration:
  • Collaborate with an interdisciplinary team of healthcare providers, including physicians, nurses, social workers, and case managers, to coordinate patient care and ensure optimal treatment outcomes.
  • Communicate and consult with specialists when needed to ensure comprehensive care for patients with complex medical conditions.
  • Facilitate referrals to other healthcare providers, ensuring proper follow-up and continuity of care.
  • Emergency and Acute Care:
  • Provide immediate care for acute conditions or medical emergencies during home visits, including triaging and stabilizing patients as needed.
  • Assess and respond to any urgent or emergent medical concerns, coordinating patient transport if necessary.
  • Patient and Family Education:
  • Educate patients and their families about medical conditions, treatment options, medications, and lifestyle changes.
  • Provide support and guidance for patients transitioning from hospital care to home care, ensuring they understand post-discharge care instructions and follow-up needs.
  • Quality Improvement & Reporting:
  • Participate in continuous quality improvement initiatives to enhance the delivery of care and patient satisfaction.
  • Report any patient safety concerns, adverse events, or changes in patient conditions to the supervising physician or clinical supervisor.
  • Attend team meetings and collaborate with clinical staff to ensure best practices are followed in patient care delivery.

Qualifications:

  • Education:
  • Master’s degree in Physician Assistant Studies from an accredited program.
  • Licensing/Certifications:
  • Current and valid Physician Assistant (PA) license in the state of practice.
  • Board Certification by the National Commission on Certification of Physician Assistants (NCCPA).
  • Basic Life Support (BLS) certification is required.
  • Advanced Cardiovascular Life Support (ACLS) certification is preferred but not required.
  • Experience:
  • Minimum of 1-2 years of experience as a Physician Assistant in primary care, urgent care, home health, or geriatrics.
  • Experience managing chronic diseases and delivering home-based care is preferred.
  • Skills:
  • Strong clinical and diagnostic skills, with the ability to work independently and make sound medical decisions in a home setting.
  • Excellent communication skills, with the ability to educate patients and families effectively and empathetically.
  • Ability to collaborate with an interdisciplinary team, providing coordinated care and ensuring positive patient outcomes.
  • Proficiency in Electronic Health Records (EHR) systems, telemedicine tools, and Microsoft Office Suite.
  • Strong organizational skills and the ability to manage a diverse patient caseload.

Physical Demands:

  • Ability to travel to patient homes using personal transportation (mileage reimbursement provided).
  • Ability to perform physical assessments, including assisting with patient mobility as necessary.
  • Occasional lifting or transferring of patients may be required.

Pay: $120,000.00 – $135,000.00 per year
