Job SummaryWe are seeking a dedicated and compassionate Primary Care Nurse Practitioner or Physician Assistant to join our healthcare team. In this role, you will provide comprehensive care to patients, focusing on health promotion, disease prevention, and management of chronic conditions. You will work collaboratively with physicians and other healthcare professionals to ensure the highest standards of patient care in a supportive environment.
Duties
- Conduct thorough physical examinations and assess patients’ health status through medical history and vital signs.
- Diagnose and manage acute and chronic illnesses, developing individualized treatment plans.
- Perform procedures such as suturing and IV infusion while adhering to aseptic techniques.
- Provide addiction counseling and support for patients dealing with substance use disorders.
- Collaborate with nursing home staff to manage the healthcare needs of residents effectively.
- Educate patients on health maintenance, disease prevention, and healthy lifestyle choices.
- Maintain accurate patient records and documentation in compliance with healthcare standards.
- Participate in quality improvement initiatives to enhance patient care services.
Skills
- Proficient in performing medical-surgical procedures and physical examinations.
- Strong knowledge of acute care practices and standards for patient management.
- Excellent communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to assess and interpret vital signs accurately.
- Experience or training in addiction counseling is highly desirable.
- Familiarity with nursing home environments and the unique needs of elderly patients is a plus.
- Strong organizational skills with attention to detail in documentation and patient care processes.
