Full Job Description

OVERVIEW:

Carolina SeniorCare is a PACE (Program of All-Inclusive Care for the Elderly) based in Lexington North Carolina which provides services to residents in Davidson, Davie, Rowan, and Iredell counties.

PACE programs operate in 33 states with the mission to provide comprehensive, personalized healthcare services including primary care to elderly people 55+ who are at risk for needing placement in a skilled nursing facility. A primary goal of PACE is to keep these vulnerable individuals in the community setting so long as they can safely be cared for in the community. This goal allows a focus more on prevention of disease than is typically done in a traditional primary care setting where treatment of disease is typically the focus.

The provider job is a clinic-based Monday-Friday 8:00 am-4:30 pm position with no after-hours, weekend, or major holiday work or call required.

QUALIFICATIONS:

1. Medical clearance for communicable diseases and up-to-date immunizations before having direct patient contact.

2. A valid driver’s license, proof of insurance, and transportation. Must be able to meet Carolina SeniorCare’s auto insurance carrier’s standards for coverage.

3. Current Physician Assistant or Nurse Practitioner license in North Carolina.

4. Current Cardiopulmonary Resuscitation (CPR) certification. [Certification training provided if needed]

5. A minimum of one year’s experience in working with the frail or elderly population.

Responsibilities:

  • Conduct thorough patient assessments and physical examinations to determine health status.
  • Maintain accurate medical records and ensure compliance with HIPAA and PACE regulations.
  • Collaborate with healthcare teams to develop and implement health plans.
