Hartford HealthCare has a new opportunity available for a Physician Assistant or Nurse Practitioner to provide primarily outpatient Lifestyle Medicine services in Cheshire and Bloomfield, CT, and potentially at additional sites.

Some highlights of this opportunity include:

  • The Physician Assistant or Nurse Practitioner will work in an ambulatory setting dedicated to lifestyle medicine and an integrated, whole-person approach to patient care.
  • Provide quality, high-touch care to all patients, including but not limited to comprehensive initial cardiovascular risk assessment, personalized health planning, diagnosis, patient education, treatment, and referrals to other services, as appropriate, using a coach-like style to patient engagement.
  • Develop educational materials aimed at teaching patients how to prevent, treat and reverse lifestyle-related chronic disease through proper nutrition, increased physical activity and mindfulness.
  • There will be a focus on program development activities, assisting the Medical Director and Program Director in refining and evaluating current practices, processes, procedures and quality improvement initiatives.
  • Assist with operations and lead cohort facilitation for the Pivio Health Improvement Program
  • Assist in possible inpatient expansion of program and other Lifestyle Medicine Program needs, as assigned
  • Adhere to the highest standards of medical practice, ethics, and professionalism at all times.
  • Contribute to open and honest ongoing communication with members and the team.
  • Attend required meetings and participate in committees, as requested.
  • Handle confidential matters concerning clinic policies and practices in accordance with HIPAA regulations.
  • Maintain strict confidentiality.
  • This opportunity is based primarily in the Hartford Region, but may cross over into other locations depending on programmatic growth.

The Hartford HealthCare Heart & Vascular Institute is a foundational institute, consistently driving Hartford HealthCare’s (HHC) approach to integrated care and clinical research. Qualifications

Eligibility to be licensed as a Physician Assistant (PA) or Advanced Practice Registered Nurse (APRN) in Connecticut is required.

Eligibility to be certified as a PA, Acute Care Nurse Practitioner (ACNP), or Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) is required.

Experience practicing as an Advanced Practitioner within cardiology is highly preferred.
