Job Summary
Nirvana Healthcare is seeking a dedicated and highly skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our newly developed Hospitalist Program. The selected candidate will provide exceptional care to our patients admitted to the acute hospital setting, ensuring seamless coordination of care from admission through discharge. This role plays a vital part in supporting Nirvana Healthcare’s mission of providing quality, patient-centered care and improving health outcomes for our diverse patient population.
Key Responsibilities
- Patient Care Management:
- Provide comprehensive care to hospitalized patients, including initial evaluations, daily rounds, progress notes, and discharge planning.
- Develop and implement patient management plans in collaboration with attending physicians and multidisciplinary teams.
- Address acute medical issues, order appropriate tests, interpret results, and adjust treatment plans as necessary.
- Care Coordination:
- Act as a liaison between the hospital and Nirvana Healthcare’s primary care and specialty providers.
- Communicate regularly with patients, families, and caregivers to ensure understanding of treatment plans and discharge instructions.
- Collaborate with case managers, social workers, and other healthcare professionals to ensure a smooth transition of care post-discharge.
- Documentation and Compliance:
- Maintain accurate and timely documentation in the electronic health record (EHR) per hospital and practice standards.
- Ensure compliance with hospital policies, state regulations, and Nirvana Healthcare’s quality measures (F-STEEP).
- Clinical Excellence:
- Deliver evidence-based care in line with current medical guidelines.
- Participate in quality improvement initiatives and contribute to the continuous improvement of the Hospitalist Program.
Qualifications
- Education:
- Master’s degree in Nursing (for NP) or a Master’s degree in Physician Assistant Studies.
- Certification:
- Current certification as a Nurse Practitioner or Physician Assistant by an accredited certifying body (e.g., ANCC, NCCPA).
- Licensure:
- Active and unrestricted license to practice in State. DEA license required.
- Experience:
- Minimum of 2 years of clinical experience in an acute care or hospitalist setting preferred.
- Skills:
- Strong clinical decision-making and diagnostic skills.
- Excellent interpersonal and communication skills.
- Ability to work collaboratively in a multidisciplinary team.
