Remote Focus specializes in Chronic Care Management, Remote Patient Monitoring, and Telehealth services, delivering high-quality care to Medicare members with chronic medical conditions. We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) for a per diem position to provide in-home medical care to our patients.

Position Overview:

The Home Care Field NP or PA will deliver in-home care services to patients, including assessments, diagnostics, treatment planning, and health education. This role involves working closely with a multidisciplinary team to ensure a high standard of care and the effective management of chronic conditions.

Key Responsibilities:

  • Conduct comprehensive in-home assessments, including medical history, physical exams, and diagnostic evaluations.
  • Develop and implement individualized care plans based on patient needs, ensuring alignment with evidence-based practices.
  • Manage acute and chronic conditions, including prescribing medications and therapies as appropriate.
  • Provide patient and caregiver education to promote understanding of care plans, medications, and preventive health measures.
  • Collaborate with interdisciplinary team members, including physicians, care coordinators, and social workers, to optimize patient outcomes.
  • Document patient encounters, assessments, and treatments accurately and promptly in the electronic health record (EHR).
  • Respond to patient inquiries and follow up on care plans to ensure continuity of care.
  • Participate in quality improvement initiatives and adhere to all regulatory and organizational standards.

Qualifications:

  • Licensure:
  • Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in New York State.
  • DEA license for prescribing controlled substances.
  • Education:
  • Master’s degree in Nursing or Physician Assistant Studies from an accredited program.
  • Experience:
  • Minimum of 2 years of clinical experience, preferably in-home care, primary care, or geriatrics.
  • Skills:
  • Strong clinical assessment and diagnostic skills.
  • Excellent interpersonal and communication abilities.
  • Ability to work independently and manage time effectively.
  • Proficiency in using electronic health records (EHR).
  • Language Skills:
  • Bilingual candidates preferred (not mandatory).

Work Environment:

  • Flexible per diem schedule based on patient needs.
  • Travel to patient homes required (reliable transportation necessary).
