About the Client
The client is an innovative US-based physicians network that has been delivering high-quality healthcare to elderly patients in assisted and independent living communities, group homes, and private homes for more than five years.
Our client has 50+ full-time healthcare clinicians (board-certified physicians, nurse practitioners, physician assistants, podiatrists, psychiatrists, psychologists, and wound-care specialists) who visit patients in the comfort of their own homes. They are also rapidly expanding the house-call network model with the patent-pending telemedicine platform via 4G/5G smartphones and tablets.

Partner Nurse Practitioner / Physician Assistant
please note, aside from the essential functions, we are looking for a promising candidate that is qualified to bring in their talent and experience on a partnership level.
Essential Functions
- Diagnose and treat patients in assisted living/group homes and skilled nursing facilities, including House Call services for common chronic diseases via “in-person” House Call and/or telemedicine visits.
- Prescribe all necessary medications and treatments
- Perform comprehensive physical examinations of patients
- Develop and implement patient management policies and procedures
- Facilitate referrals to other healthcare professionals and medical facilities
- Communicate with collaborating physicians or specialists regarding patient care
Qualifications:
- Active Arizona License in good standing
- Multiple years of experience as a Primary Care Nurse or Physician Assistant
- Must have current and active DEA
- Ability to build excellent rapport with patients
- Experience working in fast-paced environments focusing on the highest quality care
- Excellent written and verbal communication skills
- Experience working with interdisciplinary teams consisting of medical and administrative team members
- Being bilingual is a plus
Good to know:
- Providers will be required to do house call visits in assisted living, group homes and skilled nursing facilities. The successful candidate will be assigned to large facilities that will enable multiple patients to be seen with one physical visit. In return, this means the role requires minimal travelling
- Currently, applicants available to travel within the Southeast Valley or West Valley will be preferred.
- The client offers a very attractive compensation package that includes a base salary, additional per-visit percentage and – for the right candidate – company equity.
