Advanced Practice House Calls – Nurse Practitioner or Physician Assistant Position: Full-Time W2 Role Location: North Houston and Surrounding Areas, TX This position involves visiting patients in their homes for medical evaluation and management. Important Requirement: Applicants must reside in the Woodlands, Spring, Tomball, or nearby areas. Please include your home address in the application. Expected Start Date: Immediate or within 2 weeks of selection Compensation and Benefits

  • Salary: $101,000–$108,000 per year (inclusive, no transport allowance)
  • Biweekly Pay: $3,884.62–$4,153.85 via direct deposit
  • Bonuses: Performance-based bonuses available
  • Paid Time Off:
    • 6 PTO/Sick days annually
    • 9 paid holidays per year
    • 1 week paid vacation annually
    • 2 paid bereavement days for immediate family
  • Insurance: Free malpractice insurance
  • Salary Increase: 2% annually (conditions apply)

Position Details

  • Schedule: Monday–Friday, 9 AM–5 PM (No weekends or holidays)
  • Patient Load: 35–40 patients per week (7–8 patients daily)
  • Visit Duration: 25–60 minutes per patient
  • Driving Expectations:
    • Home visits within an optimized route, minimizing drive time.
    • Daily driving time is typically under 3 hours, starting from your home and ending with the last patient.

Responsibilities

  • Diagnose and manage acute, chronic, and episodic conditions
  • Prescribe medications and treatments
  • Perform thorough physical examinations
  • Develop and implement patient management plans
  • Facilitate referrals and coordinate care with home health agencies
  • Supervise and adjust home health services as needed

Support and Training

  • Providers are assigned a dedicated Medical Assistant to manage:
    • Route mapping
    • Confirming patient visits
    • Handling phone calls and messages
    • Processing and faxing orders

This support allows providers to focus on patient care.

  • Full training and ongoing clinical and administrative support are provided.

Preferred Skills and Experience

  • At least 1 year as an advanced practice provider (preferred)
  • Spanish-speaking candidates are highly desirable
  • Background in home health is preferred
  • Strong interpersonal and communication skills

Qualifications

  • Certified Nurse Practitioner or Physician Assistant license (required)
  • Flexible and able to manage daily driving within the specified schedule
  • New graduates are encouraged to apply
