IMA Medical Group is seeking to hire a full-time Physician Assistant or Nurse Practitioner who is responsible for performing comprehensive annual health assessments also known as Annual Wellness visits. These assessments are for our senior-age patients with a primary focus on assessing and closing quality care gaps, the identification and documentation of chronic medical problems, and the education and counseling of patients regarding their conditions and improvement of lifestyles that may positively impact health. IMA’s Annual Wellness Visit Providers will exercise outstanding concern for patients and provide them with appropriate healthcare services that meet state and national standards of care.
This position comes with a typical schedule of 8:30 am-5:30 pm, Monday-Friday. A DAY IN THE LIFE OF AN ANNUAL WELLNESS VISIT PROVIDER
Interviews examine and diagnose patients during an annual wellness visit. Evaluates physical, cognitive, and emotional well-being. Checks for and addresses common senior health problems (such as falls, memory concerns, depression, incontinence, pain, isolation, polypharmacy). Provides education and counseling to patients and their families that may improve their health or help to treat the health issue they are experiencing. May address medical advance care planning. Orders appropriate diagnostic tests, interprets results and communicates appropriate follow-up options to patients. May refer patient to medical specialist or another practitioner when necessary. Reviews and maintains patients’ records and medical histories in a timely fashion and records patients’ progress. May perform clinical diagnostic testing and procedures (ECG’s, pulmonary, vascular, and neurologic testing), and perform minor clinical procedures such as administering immunizations. May prescribe pharmaceuticals, other medications, and treatment regimens for identified and documented medical conditions. Discusses potential side effects of medication/treatment with patients and ensures that all allergy information is up to date before prescribing. Accurately records correct healthcare diagnoses to ensure correct billing. Stay current on the latest medical research, practices, and procedures impacting family medicine. Adheres to departmental policies, procedures, and objectives, ongoing quality improvement objectives, and safety, environmental, and infection control standards. Maintains patient confidentiality and complies with all federal and state health information privacy laws. Completes administrative assignments as requested by the Medical Director and partners with providers within the practice to ensure a collaborative environment. Actively participates in provider team meetings and performance reviews with the Medical Director. Maintains a strong and positive relationship with IMA Primary Care Providers (PCPs) to coordinate follow-up care, further diagnostic testing, and appropriate referrals of mutual patients
Requirements:
We require our candidates to possess these minimum qualifications:
- Fully licensed Physician Assistant in Florida.
- Fully state-licensed as an Advanced Practice Registered Nurse (APRN) or Advanced Registered Nurse Practitioner (ARNP).
- One year of experience Minimum of field experience (excluding clinical rotations)
- Fully bilingual (English/Spanish)
- EMR, eClinical Works experience
- Managed Care experience
- Basic Life Support certification (BLS)
