Fairview Geriatric Services seeks a board-prepared/board-certified Physician Assistant to provide care of our geriatric patients in long term care, transitional care, and assisted living facilities in the north and northeast Twin Cities Metro. The Physician Assistant, in collaboration with physicians and part of an interdisciplinary team, engages in independent decision making about health care needs of their own patient panel.
Practice Details:
- Works as part of an interdisciplinary team, engages in independent decision making about health care needs of their own patient panel.
- Meet the unique and growing needs of complex, frail, and special needs patients in care communities.
- Engage in independent decision making about health care needs and provide health care services to individuals.
- Provider Team accessibility via phone 24/7.
- Openings: 1.0 FTE.
- Adhere to the patient and family centered values of the Health Care Home.
- As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
- Fully integrated Electronic Medical Record (EPIC).
Qualifications:
- Graduate of an accredited Physician Assistant training program. BC/BE through the NCCPA.
- Licensed or ability to obtain licensure through Minnesota Board of Medical Practice.
- Ability to obtain and /or maintain DEA certification for the State of Minnesota.
- Meet threshold criteria/qualifications for Credentialing and Privileges (if required).
