Job Summary Under the general direction of the Executive Director of Operations and supervising physician(s), the Advanced Practice Provider (APP) plans, evaluates, and provides excellent care to patients in this Medical Oncology & Hematology practice. The Summit Cancer Care (SCC) APP is a Nurse Practitioner or Physician Assistant. Managing patient care across the continuum, from diagnosis through treatment and survivorship. The APP works to continually improve patient care and operations of the practice. Essential Job Responsibilities In conjunction with physicians and management, evaluates quality of care provided and recommends changes for improvement. Assesses the physical and psychological status of patients by health history, physical examination, and diagnostic studies. Ensure accurate and timely documentation of patient assessments, treatment plans, and clinical progress in the appropriate EHR (Aria, Meditech, EPIC, or Cerner) per organization for patients in the outpatient and inpatient settings. Educates patients and families about the disease process, treatment, symptom management and psychosocial response of diagnosis of cancer and related treatment(s). Provides education to staff and the public when necessary. Works closely with multidisciplinary teams to ensure continuity and quality of care. Orders and interprets diagnostic tests, including lab work, imaging, and biopsies in conjunction with supervising physicians. Assist with referrals to other specialists as necessary, ensuring seamless care transition or collaboration for patients. Maintains hospital privileges, certifications, and licensure. Participate in professional development opportunities, including conferences, workshops, tumor boards, clinical meetings, and professional organization events to enhance and stay up to date on knowledge and skills. Strong communication and interpersonal skills, with the ability to build rapport with patients and their families, staff, and outside organizations. Maintain a high level of professionalism, positivity, and teamwork mindset at all times. Ability to work independently and as part of a team. Round on patients in the hospital(s) when necessary to support physicians during weekdays, weekend days, or holidays. APPs are assigned rotating holidays and weekends to cover service in the Savannah hospitals with SCC physicians. APPs may assist physicians with hospital phone calls on their assigned weekends. Travel to satellite offices to conduct patient office visits and inpatient rounding. Cover day and late call for patients receiving treatment at all office sites. Knowledge and ability to handle prior authorizations for treatments, procedures, and imaging to include updated EHR documentation, provide additional information, or conduct Peer to Peer conversations to optimize quality of patient care. Continuously monitor and audit patient schedules for efficiency as per provider team template parameters and communicate any issues or changes needed to schedulers. Stays up to date on any learnings, policies, performance reviews, timecards, expense reports, tasks, etc. within the Paycor system. Ensure chart preparation is completed ahead of time so all information is available for productive patient office visits; obtain required documents when missing. Always monitors provider schedule on Lightning Bolt. Utilizes EHR system to e-scribe medications and utilizes the PDMP when bringing to physicians for prescription. Assist the support staff in critical decision making when scheduling new and established patients or handling incoming triage calls. Other Job Responsibilities Performs other related duties as assigned. Maintains collaborative working relationship with all other employees. Upholds strictest confidentiality. Minimal Qualifications Education Master’s degree or Doctorate in Nursing (MSN, DNP), or Physician Assistant (PA) degree from an accredited program Experience 3 – 5 years of direct patient care as an APP or Oncology RN (required) 1 – 2 years of direct patient care as an APP in Oncology / Hematology (preferred)
Certification / License BLS CPR for Healthcare Providers through American Heart Association Board certification as a Nurse Practitioner (NP) or Physician Assistant (PA) NP: Georgia State Registered Nurse License & Nurse Practitioner License PA: Georgia State Composite Medical Board Physician Assistant License
