Advanced Practice Clinician (Nurse Practitioner/Physician Assistant)
Position Summary:
Working as part of the patient care team the Nurse Practitioner will provide services to members in Clinic, Office and Urgent Care settings working within the NP scope of practice and in compliance with the collaboration protocol of designated Supervising Physician.
Essential Duties and Responsibilities include the following:
- Obtain and document all pertinent information in patient’s charts.
- Maintain patient safety and confidentiality.
- Reinforce patient education and participate in patient teaching.
- Assist with training and orientation of new personnel.
- Identify problems, provide solutions and evaluate outcome.
- Ensure appropriate handling and accurate documentation of all medications dispense or prescribed.
- Knowledgeable with referral system and ensures accurate and timely follow through with all referrals generated.
- Universal precautions utilized in all patient care.
- Perform services which require the skills and licensure of a Nurse Practitioner
- Participate in the planning and development of programs patient management and education
- Assist with initiating and maintaining peripheral lines, administering medications, maintaining and ordering of medications and supplies for infusion services in office and UC settings
- Perform procedures in all settings within NP scope of practice
- Administer treatments and medications in all settings within NP scope of practice
- Prescribe/order procedures, treatments and medications within scope of practice
- Document and maintain records within the company EMR
- Assess patients in all settings for High Risk/DM programs
- Address end of life issues through introduction of POLST
- Work with the patient care team to identify and address patient risks for admission
- All other duties as directed by management
The pay range for this position at commencement of employment is expected to be between $130,000 – $140,000 annually; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

If hired, employee will be in an “at-will position” and the Company reserves the right to modify base salary (as well as any other discretionary payment or compensation program) at any time, including for reasons related to individual performance, Company or individual department/team performance, and market factors.

Requirements:
Education and / or Experience:
- Current BLS certification.
- Valid, unrestricted Registered Nurse Practitioner License in the State of CA.
- State Nurse Practitioner Certification in appropriate specialty
Or
- Graduation from an accredited Physician’s Assistant Program.
Experience:
- 1-3 yrs NP or PA experience with confidence to practice independently
- Urgent Care experience helpful
- Managed Care experience a plus.
- Maintain collaboration agreement with contracted supervising physician
Compensation: 130k-140k
