As a Seva Medical Advanced Practice Clinician you will care for our patients, assisting them in navigating a challenging healthcare system. You’ll focus on delivering care with an emphasis on compassionate service to improve the quality of life for our patients within the comfort of their homes. You’ll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent primary care services.
Position Details
- Location: Travel required – Adult family homes, memory care and assisted living communities
- Manage complex patients, focused on reducing hospitalizations and improved health outcomes.
- Monday- Friday schedule with an ability to schedule patient appointments into condensed workdays as desired.

Requirements
- Passion for delivering compassionate care to patients with chronic illnesses
- Minimum of two (2) years’ experience as a Nurse Practitioner (NP) or Physician’s Assistant (PA)
- Current unrestricted NP/PA-C license to practice in the state for which you apply.
- Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
- Intermediate computer skills and proficiency required
- Home Office, High Speed Internet and Reliable Transportation Required.
Preferred Qualifications
- 5+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP) or Physician’s Assistant (PA-C), with experience in mobile primary care within Geriatric care, Internal Medicine, Psychiatry, and Podiatry.
- Experience in managing chronic diseases (CHF, hypertension, diabetes etc.)
- Manage population Health (hospitalization)
Job Type: Full-time
Pay: $150,000.00 – $200,000.00 per year
