- In-Home Assessment: $120
- Virtual Health Assessment: $100

Day in the life of a CCS Provider
CCS providers are true healthcare heroes connecting with members where they feel the most comfortable… in their home. Our network of NP’s and PA’s travel to members’ homes to conduct annual health evaluations for Medicare, Medicaid and Commercial health plan members. Assessments are comprehensive and at the minimum include a complete review of medications and OTCs, review of systems, physical examination and discussion of medical concerns which affect the member. With up to an hour allotted for each visit, members benefit from the enhanced 1:1 interaction with a NP or PA to gain a better understanding of their health along with talking points to bring up to their PCP in the future.
Qualifications
- Active un-encumbered license to practice nursing;
- Nurse Practitioner Certification from either the ANCC or AANP; in Family, Adult, Gerontology or Emergency Medicine.
- NP’s: Board Certified through the AANP or ANCC
- Master’s degree and successful completion of accredited physician assistant program.
- Current licensure as Physician Assistant by the state of New York
- PA-C’s: Board Certified by NCCPA
- Maintains current CPR certification
- Compliance, prior to hire, with recommended Healthcare Personnel Requirements for vaccinations and preventive testing: Hepatitis B, Influenza, MMR: Measles, Mumps and Rubella, Pertussis, Tetanus and diphtheria and acellular pertussis (Tdap), Varicella, Tuberculosis.
- Home Health exp a plus
- Must be able to effectively communicate with elderly and chronically ill patients and families;
- Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial;
- Ability to multitask;
- Excellent customer service skills;
- Bi-lingual or multi-lingual a plus.
Job Types: Part-time, PRN, Per diem
Pay: $100.00 – $120.00 per hour
