Aveon Health is a family practice specializing in diabetes care with an in-house podiatry specialist. We are dedicated to comprehensive chronic care management to improve patient outcomes and maintain high-quality care standards. We are seeking a knowledgeable Chronic Care Management Specialist to support chronic condition management, manage medication prior authorizations, and ensure compliance with insurance quality measures.
Position Summary:The Chronic Care Management Specialist will coordinate care for patients with chronic conditions, focusing on diabetes management. This role works closely with our medical team to develop care plans, manage medication prior authorizations, and enhance patient engagement. Candidates should be experienced in chronic care management, insurance quality standards, and navigating prior authorization processes.
Key Responsibilities:
- Develop and monitor chronic care management plans for patients, particularly those with diabetes.
- Coordinate with providers, including podiatry specialists, to address patient care needs.
- Handle medication prior authorization requests to ensure timely patient access to necessary treatments.
- Ensure compliance with insurance quality measures and reporting requirements.
- Conduct patient follow-up to reinforce treatment plans and improve engagement.
- Collaborate with insurance providers to verify coverage and optimize chronic care services.
- Maintain accurate documentation using electronic health records (EHR) and track patient progress.
- Review and analyze quality measures data to identify areas for care improvement.
Qualifications:
- Education: Related field (preferred); relevant experience considered.
- Experience: Minimum of 2 years in chronic care management, with a focus on diabetes preferred. Knowledge of medication prior authorization processes and insurance quality measures.
- Skills:
- Proficiency in EHR and care management software.
- Knowledge of CMS Chronic Care Management (CCM) guidelines.
- Strong organizational and communication skills with a focus on patient care.
- Ability to collaborate effectively within a multidisciplinary team.
Job Type: Full-time
Pay: $110,000.00 – $120,000.00 per year
