We are seeking a dedicated Traveling MD, Nurse Practitioner, or Physician Assistant to provide in-home patient visits in Eastvale, Riverside, CA. This role offers the opportunity to provide personalized, quality care directly to patients in their homes.
Responsibilities:
- Conduct in-home visits to assess and treat patients, ensuring a high standard of care.
- Collaborate with the broader healthcare team to deliver comprehensive and coordinated services.
- Accurately document patient information, treatment plans, and progress.
- Educate patients and their families on health management practices and preventive care.
Requirements:
- Valid medical license in the state of California.
- Board certification in your respective specialty for physicians; certification for Nurse Practitioners and Physician Assistants.
- Experience in home health, primary care, or related fields is preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and provide high-quality care in a home setting.
