Nurse Practitioner/Physician Assistant Primary Care

Job Summary

The Nurse Practitioner/Physician Assistant will provide primary care services to patients in an assisted living/skilled nursing facilities. The successful candidate will work collaboratively with the facility’s medical director and other healthcare professionals to provide high-quality care to patients.

Responsibilities

• Conduct initial and ongoing assessments of patients’ health status
• Develop and implement care plans based on patients’ needs and goals
• Prescribe medications and treatments as appropriate
• Order and interpret diagnostic tests
• Provide patient education and counseling on health promotion and disease prevention
• Collaborate with other healthcare professionals to ensure coordinated and comprehensive care
• Maintain accurate and up-to-date medical records
• Participate in quality improvement initiatives

Qualifications

• Current licensure as a Nurse Practitioner or Physician Assistant in the state of Kentucky
• Master’s degree in Nursing or Physician Assistant Studies
• Certification as a Nurse Practitioner or Physician Assistant
• Experience in primary care, preferably in an assisted living or long-term care setting
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong problem-solving and critical thinking skills
