Title: Wound Care Nurse Practitioner/Physician Assistant
Salary: 800$/day + performance
Location: Contra Costa

We are seeking a dedicated and experienced Wound Care Nurse Practitioner/ Physician Assistant to join our healthcare team. As a vital member of our team, you will provide comprehensive wound care management to patients, utilizing your expertise in assessing, diagnosing, and treating acute and chronic wounds.
Responsibilities:
- Conduct thorough assessments of patients’ wounds, including wound measurements, tissue assessment, and identification of underlying conditions.
- Develop and implement individualized treatment plans tailored to patients’ specific wound care needs, including debridement, dressings, and adjunctive therapies.
- Collaborate with interdisciplinary healthcare team members to ensure comprehensive patient care and treatment outcomes.
- Educate patients and caregivers on proper wound care techniques, prevention strategies, and lifestyle modifications.
- Monitor patients’ progress and adjust treatment plans accordingly to promote optimal healing outcomes.
- Maintain accurate and up-to-date documentation of patient assessments, interventions, and progress notes.
Requirements:
- Current certification as a Nurse Practitioner.
- Master’s degree in Nursing from an accredited institution.
- Active state licensure as a Nurse Practitioner.
- Minimum of 2 years of experience in wound care management, with proficiency in wound assessment, treatment modalities, and evidence-based practice.
- Strong interpersonal and communication skills, with the ability to effectively collaborate with patients, families, and healthcare team members.
- Commitment to providing compassionate, patient-centered care and promoting positive patient outcomes.
Preferred Qualifications:
- Certification in wound care (CWCA, CWCN, or equivalent).
- Experience working in an outpatient wound care clinic or hospital setting.
- Proficiency in electronic medical record (EMR) documentation systems.
