Vivo HealthStaff is recruiting Nurse Practitioners in Fresno, California. This is a permanent full-time position.
Details:
- Provide comprehensive healthcare to patients with complex and chronic care needs.
- Job is hybrid ( some days remote and some days doing home visits)
- Conduct initial and annual face-to-face patient visits at home; will be seeing 5-7 patients per day; 5 if doing home visits and 7 if tele visits
- Perform health risk assessments, including medical history review, symptom evaluation, and physical examinations.
- Develop care plans for chronic disease management.
- Collaborate with a multidisciplinary team to ensure quality patient care.
Requirements:
- Master’s degree from an accredited NP Program or PA program
- One (1) year of prior clinical or home care experience
- Previous EMR experience preferred
- Active California state Nurse Practitioner or Physician Assistant license
- Active RN License and Furnishing number (Must, upon hire) NP only
- Active Nurse Practitioner Board Certification/Physician Assistant Certification
- NPI Number, DEA, Valid BLS
- Valid California driver’s license and current automobile insurance
- Experience in the care of older adult (geriatric) patients preferred
- Excellent administrative, organizational, and communication skills required
