Full Job Description

A progressive, value-based healthcare organization is seeking Physician Assistants and Nurse Practitioners with a passion for serving the geriatric population, to join their team! This company specializes in caring for patients with complex, chronic conditions. They also have a great reputation for providing whole health to their patients.

We are currently hiring 2 full-time and permanent NP’s or PA-C’s in the Greater Lanesville Areas for 2 different patient panels in:

  • Decatur
  • Springfield

Nurse Practitioner (NP) or Physician Assistant (PA-C) Responsibilities Include:

As a Field Nurse Practitioner (NP) or Physician Assistant (PA-C), you will be responsible for improving patients’ quality of life from the comfort of their homes. The Nurse Practitioner or Physician Assistant will work together with the interdisciplinary team to improve patients’ medical, social, and emotional health.

  • Collaborate with the interdisciplinary team managing the health and wellbeing of patients living in their homes
  • Provide primary care to patients in the home and urgent care as needed
  • Deliver palliative care, and when needed, end of life care to patients and families
  • Perform initial patient screenings/physical examinations and create appropriate assessment/care plans
  • Order, interpret, and analyze diagnostic tests and determine the need for follow-up appointments and further treatment options
  • Prescribe and manage medications and treatments as appropriate, including but not limited to psychopharmacological agents

Schedule: Monday – Friday, 8:00am – 5:00pm

  • No nights, No weekends
  • Recently made changes to ensure that the charting and documentation is built into the schedule, which has made a significant difference in the work/life balance aspect of the job!!

Annual Compensation: $120,000 to $140,000
