Job Summary:

We are seeking a highly skilled and motivated Physician Assistant (PA) to join our innovative Hospital at Home program. This role is focused on conducting virtual patient assessments to determine eligibility and provide ongoing care for patients admitted to our at-home hospital program. The ideal candidate will have a strong background in internal medicine, emergency medicine, or as a hospitalist, with exceptional communication skills and experience in telemedicine. This role will support field-based Physician Assistants who are providing in-person patient care in the homes of the patients admitted to the program.

Key Responsibilities:

  • Virtual Patient Assessments:
  • Conduct comprehensive virtual assessments to evaluate patients’ medical conditions, history, and current symptoms.
  • Review and analyze patients’ medical records, test results, and other relevant documentation to inform care decisions.
  • Determine patient eligibility for the Hospital at Home program based on clinical criteria and program guidelines.
  • Patient Management:
  • Develop and implement individualized care plans in collaboration with the multidisciplinary team.
  • Monitor and manage patients remotely, including coordinating with home health providers and other healthcare professionals.
  • Provide ongoing virtual follow-up assessments to track patient progress and make necessary adjustments to care plans.
  • Communication and Coordination:
  • Communicate effectively with patients, families, and caregivers to provide education about the program and ensure understanding of care plans.
  • Coordinate with primary care physicians and specialists to ensure continuity of care and address any emerging health issues.
  • Maintain accurate and up-to-date patient records and documentation in compliance with regulatory requirements and program standards.
  • Schedule and Compensation:
  • 7 days on, 7 days off 7AM -5PM PST
  • Salary range is dependent upon years of industry experience and educational credentials; payroll schedule is weekly.

Qualifications:

  • Education & Certification:
  • Master’s degree in Physician Assistant Studies from an accredited program.
  • Certification by the National Commission on Certification of Physician Assistants (NCCPA).
  • State licensure to practice as a Physician Assistant in California
  • Valid DEA License
  • Experience:
  • Minimum of 5 years of clinical experience in internal medicine, emergency medicine, or as a hospitalist
  • Previous experience with telemedicine or virtual patient care is highly desirable.
  • Experience in a hospital-at-home or home health setting is a plus.
  • Skills:
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and interpersonal skills, with the ability to provide clear instructions and support in a virtual setting.
  • Proficiency in using telemedicine platforms and electronic health records (EHR) systems.
  • Ability to work independently and manage multiple patient cases simultaneously.

Job Types: Full-time, Contract

Pay: $190,000.00 – $210,000.00 per year
