Job Description: Home Healthcare Physician Assistant in Concord, CA
Overview:

As a Home Healthcare Physician Assistant, you’ll play a vital role in this mission, providing high-quality care and ensuring patients receive the best possible treatment in the comfort of their homes.
Local Pay Rate: $90-100/hr (paid weekly, eligible for benefits if full-time)
Shifts & Schedule: 4×10 hour shifts per week, totaling 40 hours. Rotating weekends and in-home visits required.

Responsibilities:
- Serve as the key liaison between patients, families, and physicians.
- Conduct evaluations and develop acute care plans for home-based patients.
- Deliver inpatient-level care at home, prioritizing patient comfort and safety.
- Communicate effectively with patients and families, integrating their values into care plans.
- Build collaborative relationships with healthcare providers.
- Ensure compliance with safety protocols and quality standards.
Qualifications:
- Master’s degree and current PA-C license.
- Certification in acute care, ambulatory care, or a related field preferred.
- Minimum of five years of PA-C experience.
- Flexibility to work across shifts and days.
- Strong communication, critical thinking, and empathy skills.
