Physician Assistant/Nurse Practitioner – Program Manager, Palliative Care

Position: Physician Assistant/Nurse Practitioner – Program ManagerLocation: Brooklyn, NYSchedule: Monday – Friday, 9 AM – 5 PM (40 hours/week)

Overview:The Advanced Illness Program, based in Brooklyn, NY, is seeking a highly experienced and compassionate Physician Assistant or Nurse Practitioner to join the team as a Program Manager. This role offers an opportunity to manage the Center’s Advanced Illness Management (AIM) program and Age Friendly Health System Projects, contributing to the organization’s mission of providing quality palliative care services. Key Responsibilities:

  • Oversee day-to-day operations of the AIM program and Age Friendly Health System Projects.
  • Collaborate with the Director of Business Intelligence and Value-Based Management, AVP of Advance Practice Providers, and the Social Work Director to ensure program goals are met.
  • Ensure smooth coordination between healthcare providers, patients, and families, focusing on advanced illness and palliative care.
  • Manage and evaluate program performance, implementing improvements to ensure efficient and quality care delivery.
  • Educate and support healthcare providers on best practices in palliative care and age-friendly health systems.
  • Engage with interdisciplinary teams to develop patient-centered care plans that align with palliative care principles.

Qualifications:

  • Completion of an accredited Physician Assistant or Nurse Practitioner program with current NYS licensure and registration.
  • Certification from ANCC, AANP, or AAPA.
  • ACLS and BCLS certification.
  • Strong interpersonal and English verbal/written communication skills.
  • Leadership skills with the ability to influence and guide a dynamic team.
  • Problem-solving ability, with knowledge of cost accounting concepts.
  • Proficiency in MSOffice (Word, Excel, Outlook).

Preferred Qualifications:

  • Experience in palliative care or related fields.
  • Palliative care certification.
  • 1-2 years of experience as a Physician Assistant or Nurse Practitioner.
