Advanced Practice Provider: Physician Assistant / Nurse Practitioner

Revive Pathway is a Tribally owned clinic that treats a diverse population of patients, including Native and Non-Native people. We have the opportunity to provide whole person care, while managing patient substance use disorders, primary care, infectious disease, and mental health needs. We are currently seeking an Advanced Practice Provider: Physician Assistant / Nurse Practitioner to support our clinic in El Cajon, CA. This position, reporting to the Clinic Medical Director, is an excellent opportunity for someone excited to make an immediate and lasting impact in a growing company.

Position Overview:

Revive Pathway is looking for a Board-Certified Physician Assistant / Nurse Practitioner to guide patients as they make their way to a better life. This position will require you to utilize a flexible approach to challenges with each patient. Must be willing to cross-train and do what it takes to help the team each day. Expertise and humility can be a rare combination, but it’s the secret to a successful team. You will play an active and integral role in optimizing evidence-based care for our patients and in supporting them in reaching their goals. You will also ensure the provision of patient-centered treatment for substance use disorder. In turn, you will be supported by a compassionate team of employees who get up each day to fight the battle against this health care crisis in our community. Minimum Requirements:

If Nurse Practitioner:

  • Active RN License
  • Active board certification as an NP

If Physician Assistant

  • Active PA board certification AND
  • Active PA license

All Nurse Practitioners and Physician Assistant applicants must have:

  • Active DEA license or ability to obtain one prior to working with us.
  • Active American Heart Association (AHA) Basic life support or Advanced Cardiac Life Support
  • Ability to pass drug and background screening.
  • Ability to provide health and TB clearance.
  • Licensure in good standing and adhere to all applicable legal requirements.

Preferred Requirements:

  • Active American Heart Association (AHA) ACLS
  • 2 years of recent/previous work experience in substance use disorder setting.

Clinical Knowledge:

  • Demonstrates knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of substance use disorders as well as medical conditions.
  • Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical problem.
  • Orders appropriate diagnostic studies and correctly interprets the results.
  • Utilizes critical thinking to synthesize and analyze collected data.
  • Develops, implements, evaluates, and alters the plan of care as needed.
  • Manages general medical and addiction medicine based on knowledge of the indications, contraindications, side effects, interactions and adverse reactions of pharmacologic agents and other relevant treatment modalities.
  • Counsels and educates patients and families.
  • Demonstrates proficiency in procedural skills pertinent to practice area.
  • Maintains accurate, complete, concise, and timely documentation in the electronic medical record.

Patient Care:

  • Provides care that is patient and family centered, compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease.
  • Facilitates consistent, coordinated care and clear communication among all members of the healthcare team.
  • Incorporates the patient and family in care planning and treatment across settings.
  • Serves as an advocate for patients and families.

Communication and Interdisciplinary Collaboration:

  • Communicates and collaborates effectively with physicians/team members and health related agencies.
  • Works effectively with multidisciplinary team members.
  • Promotes mutual respect and trust working within a team model.
  • Communicates effectively with patients and families, incorporating gender, age, culture, race, religion, disabilities, and sexual orientation.

Professionalism:

  • Follows ethical principles regarding patient confidentiality.
  • Participates in education of students and other health care professionals.
  • Maintains effectiveness where circumstances and priorities are changing. Is flexible, resilient and change oriented.
  • Welcomes and participates in opportunities to advance the mission of the clinic through service, science, and social responsibility.
  • Participates in organizational and departmental meetings, educational opportunities, committees, and workgroups as necessary.
  • Participates in quality initiative/performance activities.

Specific Responsibilities:

  • Conducts comprehensive health history and physical assessment of patients with acute health problems.
  • Performs history and physical, develops, manages, and modifies treatment plans.
  • Dispense Medications and Maintain records for controlled substances.
  • Participates in daily huddle and collaborates with multidisciplinary health care team as needed to adjust plans of care.
  • Facilitates referrals to appropriate community health facilities, agencies, and resources when indicated.
  • Comply with applicable state and federal drug laws as regulated by the relevant applicable jurisdiction.

Proficiency in the below procedures preferred:

Perform routine clinical procedures independently including, but not limited to, the following:

  • Laceration repair with sutures, staples, or Dermabond
  • Splinting fractures
  • Foreign body removal
  • Incision and drainage
  • Suture/staple removal
  • Pelvic exams

Physical Demands and Work Environment:

While performing the duties of this classification, the employee is frequently required to stand, walk, sit, stoop, kneel, bend, use hands to keyboard or type, handle materials used in performing the essential functions of the classification, and reach with hands and arms. The employee will be asked to perform repetitive behaviors (e.g., typing and data entry). The employee must occasionally lift and/or move up to 25 pounds (think boxes of paper). Specific vision abilities required by this classification include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Sufficient clarity of speech and hearing abilities required by this classification includes those which permit the employee to discern verbal instructions and communicate effectively in person, by telephone. While performing the duties of this classification, the employee works in an office setting where the noise level in the work environment is usually moderate. The employee must also analyze and evaluate situations and adopt effective courses of action.
