Location: Rochester, New York (Monroe County)

Salary: $125,000 – $140,000

Sign-On Bonus: $30,000

Position Overview:We are seeking a passionate Nurse Practitioner or Physician Assistant to join our care team in New York. In this role, you’ll be at the forefront of providing comprehensive, in-home care to patients, managing their health journeys from start to finish. Your expertise will play a vital role in improving patient outcomes and ensuring continuity of care.

What You’ll Do:• In-Home Assessments (IHA): Conduct thorough in-home assessments, reviewing medical history, performing physical exams, and managing medications.• Patient Care: Evaluate and diagnose patients, creating personalized treatment plans using evidence-based practices.• Chronic Disease Management: Help patients manage chronic conditions like diabetes, hypertension, and cardiovascular disease while educating them on proper care.• Medication Management: Prescribe medications and adjust dosages while educating patients on their use and possible side effects.• Preventive Care: Encourage preventive measures like screenings and immunizations to promote long-term health.• Patient Education: Empower patients by educating them about their health conditions and best practices for self-care.• Collaborative Care: Work closely with a team of healthcare professionals to ensure seamless and coordinated patient care.• Primary Care Provider: Serve as the primary healthcare provider for patients, overseeing their overall health.• Accurate Documentation: Keep detailed and up-to-date electronic health records (EHR) to track patient care.• Exceptional Bedside Manner: Build strong, trusting relationships with patients and their families by showing genuine care and empathy.• Regular Follow-Up: Schedule follow-up visits to assess treatment effectiveness and make necessary adjustments.• Quality Assurance: Participate in quality improvement programs to maintain the highest standards of care.• On-Call Flexibility: Be available for after-hours on-call duty as needed and conduct IHA and longitudinal care visits.• Travel: Visit patient homes within a designated region.• Adaptability: Handle other duties as assigned to meet the evolving needs of the business.

What We’re Looking For:• Licensure: Current, unrestricted Nurse Practitioner license in New York or Physician Assistant in the state of New York.• Certification: Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner certification.• Experience: Preferred 2+ years in primary care or longitudinal care, with eligibility for autonomous provider status within six months.• Driving Record: Valid driver’s license with no restrictions, and no DUI or felony driving conviction in the past 7 years.• Certifications: Basic Life Support (BLS) and DEA license required.• Skills: Strong clinical assessment, diagnostic skills, and proficiency in EHR systems.

Bonus Qualifications:• Experience with HEDIS/STAR measures.

  • Home-based care or visit experience.
  • HCC documentation experience.

Your Schedule:• Standard Hours: Monday to Friday, 8:00 AM to 5:00 PM, with a one-hour lunch break.• Patient Visits: Minimum of 25 visits per week.
