Overview: We are seeking dedicated Nurse Practitioners (NP) or Physician Assistants (PA) with a passion for geriatric care to join our team of professionals. This position offers the opportunity to provide comprehensive medical care, manage chronic conditions, and collaborate closely with interdisciplinary teams to ensure high-quality patient outcomes in a supportive, professional and dynamic environment.

Responsibilities & Qualifications
  • Patient Care:
    • Conduct thorough assessments, develop care plans, and perform routine follow-up evaluations for patients in the rehab and long-term care units.
    • Manage acute and chronic medical conditions, including diabetes, hypertension, respiratory diseases, and neurological disorders.
    • Prescribe appropriate medications and treatments, adjusting them as necessary based on patient response and evolving care needs.
  • Collaboration:
    • Work closely with attending physicians, nursing staff, physical therapists, and other healthcare professionals to coordinate comprehensive patient care plans.
    • Participate actively in interdisciplinary team meetings to discuss patient progress, treatment plans, and discharge planning.
  • Documentation and Compliance:
    • Maintain accurate and up-to-date medical records, ensuring compliance with facility policies, state regulations, and billing requirements.
  • Complete documentation in a timely manner, including progress notes, medication orders, and discharge summaries.
  • Patient Education:
    • Educate patients and their families on disease prevention, treatment options, medication adherence, and lifestyle modifications to promote overall well-being.
    • Provide counseling and support to patients facing complex medical decisions or challenges.
  • Quality Improvement and Training:
    • Participate in quality improvement initiatives, clinical audits, and performance improvement projects to enhance patient care outcomes and facility practices.
    • Mentor and provide clinical guidance to nursing staff and other healthcare providers as needed.
Qualifications:
  • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in New York.
  • Certification as an NP or PA from an accredited program.
  • Previous experience in geriatrics & primary care.
  • Strong clinical assessment skills and the ability to make autonomous clinical decisions.
  • Excellent communication skills and the ability to collaborate effectively within a multidisciplinary team.
  • Proficiency in electronic health records (EHR) systems and medical documentation

Compensation:
  • $130,000 – $200,000
