About Us:
We are a Texas-based private outpatient clinic specializing in comprehensive wound care, serving patients in long-term care and skilled nursing facilities across the state. We are committed to improving the lives of individuals with chronic and acute wounds through evidence-based treatment and compassionate care.
We are currently seeking an experienced and self-motivated Physician Assistant (PA) or Nurse Practitioner (NP) to join our growing team. This position is ideal for providers who enjoy independence, clinical autonomy, and travel.
Position Overview:
The Wound Care PA/NP will travel to assigned nursing homes and long-term care facilities throughout Texas to provide on-site wound assessments, treatments, and follow-up care. You will work closely with facility staff and collaborating physicians to ensure high standards of wound management and healing outcomes.
Responsibilities:
- Perform on-site wound assessments, debridement, and treatment procedures
- Develop and implement individualized wound care plans in collaboration with facility staff
- Accurately document clinical encounters using EMR systems
- Monitor wound progress and adjust treatment plans as necessary
- Educate facility staff and patients on wound prevention and care
- Communicate regularly with the supervising physician and care teams
- Maintain compliance with HIPAA, infection control, and facility policies
Requirements:
- Active and unrestricted Texas license as a PA or NP
- National certification (NCCPA for PAs or AANP/ANCC for NPs)
- Wound care certification (e.g., WCC, CWCN, CWS) preferred but not required
- Minimum of 1 year of clinical experience in wound care, geriatrics, or post-acute care
- Must be able and willing to travel extensively throughout the RGV
- Reliable transportation and valid driver’s license required
- Strong organizational and communication skills
- Proficiency with electronic medical records
Compensation and Benefits:
- Competitive salary: $100,000 – $125,000 annually (based on experience and qualifications)

Work Environment:
- This is a hybrid position that requires daily travel to skilled nursing facilities, rehabilitation centers, and long-term care homes throughout assigned regions in the RGV.
