Job Summary

We are seeking a dynamic and compassionate Nurse Practitioner to join our healthcare team, dedicated to delivering exceptional patient care across diverse clinical settings. In this role, you will serve as a vital member of the healthcare delivery system, providing comprehensive assessments, diagnosing conditions, and developing personalized treatment plans. Your expertise will help improve patient outcomes through proactive management, health coaching, and collaborative practice. This position offers an exciting opportunity to work with a broad patient population, including pediatrics, geriatrics, and individuals with complex medical needs, utilizing advanced clinical skills and a wide range of medical procedures.

Responsibilities

  • Conduct thorough physical examinations and patient assessments to determine health status and diagnose medical conditions.
  • Prescribe medications and treatment plans in accordance with state regulations and collaborative practice agreements.
  • Manage acute and chronic illnesses across various specialties such as internal medicine, geriatrics, pediatrics, behavioral health, and emergency care.
  • Utilize electronic health record (EHR) systems like Epic, Cerner, or eClinicalWorks for accurate documentation of patient encounters and medical records.
  • Coordinate care through case management strategies and discharge planning for inpatient or post-acute patients.
  • Engage in telehealth consultations to extend access to quality healthcare services remotely.
  • Collaborate with multidisciplinary teams to ensure comprehensive patient-centered care while adhering to HIPAA regulations and safety protocols.

Skills

  • Extensive experience working with diverse patient populations including children, seniors, individuals with disabilities, and those requiring hospice or palliative care.
  • Proficiency in EMR/EHR systems such as Epic, Athenahealth, Cerner or eClinicalWorks for efficient documentation and data management.
  • Strong clinical skills in procedures like venipuncture, sterile processing techniques, tube feeding management, gastrostomy care, and respiratory support including ventilator management.
  • Knowledge of pharmacology for prescribing medications within DEA regulations; familiarity with CPT coding for billing purposes
  • Ability to perform diagnostic evaluations using sonography or laboratory testing; experience with infectious disease care is a plus.
  • Skilled in triage assessment during urgent care or emergency room situations; capable of managing acute pain and airway emergencies.
  • Excellent communication skills for patient education on health coaching topics such as nutrition, medication adherence, and chronic disease management.
  • Familiarity with Workers’ Compensation law and utilization review processes; experience in outpatient clinics or medical-surgical units is advantageous.
