Job Overview

Community Wellness is seeking a compassionate, motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing outpatient healthcare practice in Millington, Tennessee.

We are looking for a provider who enjoys building relationships with patients and providing personalized, evidence-based care in a supportive, patient-centered environment.

The provider will see a combination of primary care, acute/sick visits, wellness visits, preventive care, weight management, medication follow-ups, and other outpatient services.

This position is ideal for a provider who is comfortable working with a high degree of autonomy within their scope of practice while collaborating appropriately with our healthcare team.

A collaborating physician is already established with the practice. The practice will cover the associated collaborating physician fee. The selected provider will be required to enter into and sign the applicable collaborative/supervisory agreement with the practice’s established collaborating physician as part of onboarding.

Responsibilities

• Perform comprehensive patient histories and physical examinations
• Evaluate, diagnose, and treat common acute and chronic conditions
• Develop individualized treatment and follow-up plans
• Prescribe medications in accordance with Tennessee law and applicable collaborative/supervisory requirements
• Provide primary, preventive, and wellness care
• Evaluate and treat patients for common sick visits and minor acute conditions
• Manage weight-management patients, including appropriate medication management and follow-up
• Order, review, and interpret laboratory testing
• Perform appropriate in-office procedures, injections, and specimen collection
• Educate patients regarding medications, treatment options, lifestyle modifications, and preventive care
• Provide telehealth visits when clinically appropriate
• Coordinate referrals and collaborate with specialists and other healthcare professionals when necessary
• Maintain accurate, timely, and thorough medical documentation
• Utilize appropriate ICD-10 and CPT coding
• Maintain HIPAA compliance and patient confidentiality
• Provide appropriate follow-up and continuity of care
• Contribute to the continued development and growth of the practice and its medical services

Qualifications

• Current Tennessee Nurse Practitioner or Physician Assistant license, or ability to obtain Tennessee licensure
• Current national board certification as applicable
• Current prescriptive authority as applicable to licensure
• Willing and able to enter into the required collaborative/supervisory agreement with the practice’s established collaborating physician
• Strong clinical assessment and decision-making skills
• Comfortable evaluating and managing common outpatient acute and chronic conditions
• Excellent patient communication and bedside manner
• Ability to practice independently within scope while collaborating appropriately with the healthcare team
• Strong organizational and medical documentation skills
• Familiarity with electronic health record (EHR/EMR) systems
• Knowledge of ICD-10 and CPT coding preferred
• Dependable, professional, and committed to continuity of patient care

Preferred Experience

Experience in one or more of the following areas is preferred but not required:

• Family medicine or primary care
• Urgent care
• Preventive and wellness medicine
• Weight management
• Women’s or men’s health
• Hormone management
• Functional or integrative wellness
• IV therapy
• Injectable treatments
• Telehealth
• Minor outpatient procedures
