We seek a full-time Advanced Practice Provider (NP/PA) to join our growing Physical Medicine & Rehabilitation (PM&R/physiatry) team serving patients in skilled nursing and rehabilitation facilities in Charleston, SC. This is a highly flexible position and one of the best opportunities out there for new grads, clinicians new to SNFs/post-acute care—we’ll train you the right way—or clinicians with SNF experience.

Core responsibilities
- Perform admission, follow-up, and discharge visits with attention to function, mobility, and rehab goals
- Diagnose and manage common post-acute and chronic conditions (e.g., debility, post-surgical recovery, orthopedic and neurologic conditions, and medical comorbidities)
- Coordinate rehab and medical plans with PT/OT/SLP, nursing, and social work
- Identify and address common SNF issues such as falls, wounds, pain, infections, and acute changes in condition
- Complete timely, high-quality documentation in PCC (or similar EHR)
Job Type: Full-time
Pay: $105,000.00 – $145,000.00 per year
