Job Title: Field Nurse Practitioner (NP) or Physician Assistant (PA-C)
Location: Portland, Oregon
Compensation: $115,000 – $135,000/year + Quarterly Bonus (10%)
Sign-On Bonus: $10,000
About the Role:
A highly reputable, value based, in-home provider organization, is seeking Nurse Practitioners and Physician Assistants to join their team in the New York market. As a Field Nurse Practitioner (NP)/ Physician Assistant (PA-C), you will play a vital role in enhancing the quality of life for patients by providing comprehensive care in the comfort of their homes. You will be visiting anywhere from 6-8 patients per day providing primary, urgent, and palliative care to the adult geriatric patient population. You will work as part of a collaborative interdisciplinary team, addressing the medical, social, and emotional needs of your patients. This is a full-time position offering flexible, Monday-to-Friday hours with minimal telephonic on-call duties.
Responsibilities:
- Collaborate with an interdisciplinary team to manage patients’ health and well-being.
- Provide in-home primary care and urgent care as needed.
- Deliver palliative care, including end-of-life care, to patients and families.
- Conduct initial patient assessments and create care plans.
- Order and interpret diagnostic tests and determine the need for follow-up appointments or further treatments.
- Prescribe and manage medications, including psychopharmacological agents.
Schedule:
- Full-time: Monday – Friday (8 AM – 5 PM)
- Minimal telephonic on-call duties
Compensation & Benefits:
- Competitive base salary: $115,000 – $135,000/year, based on experience
- 10% bonus paid quarterly

Qualifications:
- Unrestricted, active NP/PA-C license in the state of Oregon
- Board certified
- Minimum of 2 years of NP/PA-C experience
- Valid driver’s license
