Position Summary: We are seeking a compassionate and experienced Functional Medicine Provider, specifically a Nurse Practitioner (NP) or Physician Assistant (PA), to join our integrative health team. The ideal candidate will have expertise in functional medicine practices, including hormone management, menopause support, and comprehensive patient-centered care. The candidate should be an entrepreneurial spirit that can demonstrate capabilities to design initiatives and manage program implementation. Experience in Family Medicine or Primary Care is a must and a certification with the Institute for Functional Medicine (IFM) or related training is preferred. Responsibilities:
- Conduct comprehensive patient assessments, including in-depth health history, lifestyle, and nutrition evaluation, to uncover root causes of health concerns.
- Develop personalized treatment plans focused on functional medicine principles, incorporating nutrition, lifestyle modifications, supplements, and hormone therapies as appropriate.
- Participate in the ideation, development, and implementation of Functional Medicine programs with a focus on integration into the larger practice.
- Provide ongoing management and education for patients experiencing menopause and hormone imbalances, supporting them through lifestyle changes and hormone replacement therapies.
- Educate and counsel patients on functional medicine practices, including preventative health, nutrition, and wellness strategies.
- Collaborate with the healthcare team to ensure a holistic approach to patient care, sharing insights and updates on patient progress.
- Monitor and evaluate the effectiveness of treatments, making adjustments based on patient responses and clinical outcomes.
- Maintain detailed, accurate, and timely patient records in the EMR system in compliance with HIPAA and practice standards.
Qualifications:
- Licensure: Active Nurse Practitioner (NP) or Physician Assistant (PA) licensure in Ohio.
- Experience: Minimum of 3+ years of Family Medicine or Primary Care clinical experience required. 1-2 years in functional medicine, hormone management, and/or women’s health preferred.
- Certification: IFM or A4M certification is highly desired. Candidates with equivalent functional medicine training or partial certification will be considered.
- Skills: Proficient in patient-centered care, with a strong understanding of functional medicine, hormone therapy, and lifestyle-based treatment.
- Communication: Excellent interpersonal skills, with an ability to educate, empathize, and communicate effectively with patients.
