Full Job Description
Established Nephrology practice with offices in Kettering and Beavercreek, Ohio seeking a full time Nurse Practitioner or Physician’s Assistant to join our care team! We have an immediate opening for a compassionate, independant and driven APP who is looking to provide comprehensive patient care in a variety of settings.
APP will provide a combination of patient care in the outpatient clinic setting, the outpatient dialysis setting and some in-patient rounds. Areas of focus for this role will be the outpatient management of chronic kidney disease and includes patients with end stage kidney disease maintained on dialysis.
APP is expected to:- Work in collaboration with physicians and other healthcare professionals to provide care across the continuum of care for assigned patients.- Obtain medical histories, conduct physical examinations and assess and determine differential diagnoses.- Adjusts patient care plans by reviewing and monitoring treatments as needed.- Provide one on one patient education sessions on disease management, lifestyle and preparing for dialysis.- Interpret and evaluate patient data, including lab results, for the purpose of determining management and treatment of patients.- Refer patients who need further treatment, evaluation or assistance to various medical providers and community agencies.- Prescribes, recommends and regulates pharmacologic and non-pharmacologic therapies.- Collaborate with and participate in patient-focused value based care programs.
The ideal candidate demonstrates leadership through involvement on team projects, education, and research and actively seeks to improve the practice through performance improvement initiatives. Must have excellent verbal and written communication skills and possess strong computer skills. Must be able to work collaboratively on a multi-disciplinary team. Must be flexible and willing to handle change.

Salary is negotiable. Traveling will be required. We are willing to train on the specialty.
