We’re seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide primary care to residents in a Skilled Nursing Facility (SNF).
This is an excellent opportunity for a provider who enjoys building long-term patient relationships in a supportive, collaborative environment with a consistent Monday–Friday schedule.
What You’ll Do
- Provide on-site primary care to 25–30 residents per day
- Perform assessments, diagnose, and treat acute and chronic conditions
- Order and interpret labs and diagnostic tests
- Prescribe medications and develop individualized treatment plans
- Collaborate with nursing staff and interdisciplinary teams
- Educate residents and families on treatment plans
Qualifications
- Active Massachusettes license
- Minimum 6 months of Skilled Nursing Facility (SNF) or Long-Term Care (LTC) experience (Preferred)
- Strong clinical judgment and communication skills
- Passion for providing high-quality care to older adults
