- Minimum 3600 hours of NP/PA experience

Position Classification:1099 – Independent Contractor
Required Licensure:NY State Board Certified Nurse Practitioner / Physician Assistant, NYS Medicaid Number
Required Experience:Two (2) or more years of working experience as a Nurse Practitioner/Physician Assistant (3600 Practice Hours)
JOB SUMMARY:
This position supports the statewide Independent Practitioner Panel that issues physician orders for individuals who have undergone a community health assessment and are eligible for community-based long-term care services and supports. The incumbent will review the completed community health assessment tool in the NYS Uniform Assessment System, to complete a physician’s order. Assessments can be conducted via telehealth or face-to-face interactions.
JOB RESPONSIBILITIES
1. Responsible for reviewing the community health assessment and evaluating consumers to determine medical stability for receiving long term care services and supports at home.
2. Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments.
3. Assess cases presented and utilize clinical competencies to review and validate assessment outcomes.
4. Work collaboratively with clinical management to ensure clinical quality is met for each assessment.
5. Demonstrate communication skills necessary to interpret age-specific responses.
6. Participate in performance improvement, quality assurance activities and related meetings.
7. Meet all standards established for this position as outlined in the in-home observation/clinical evaluation tool for this position.
8. Perform other duties as may be assigned by management.
Pay: $90.00 – $105.00 per hour
