Job Description
PURPOSE OF POSITION This position adds an innovation role to improve team-based care in adult primary care clinic. This role supports PCPs and Care Teams in coordinating and managing patients in between episodes of care.
AREAS OF RESPONSIBILITIES Medical Primary Care
- Prior Authorizations
- Medication refills
- Consult recommendation ordering and follow up
- Forms follow up and completion
- InBasket triage
- Abnormal results follow up and management
- Infusion center related consents and orders
- PCP Vacation InBasket coverage
- Patient message response
Minimum Qualifications
1. Current New York State registration as a Physician Assistant; and, 2. Current certification issued by the National Commission on Certification of Physician Assistants (NCCPA); and, 3. Two years of full time, satisfactory clinical experiences as a Physician Assistant.
