Overview:
We are currently seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) with wound care experience or (interest in wound care) to join our healthcare team in the Kansas City area. This isn’t your typical SNF role. You’ll be at the forefront of our acute intervention program, working with clinical autonomy and physician support to manage complex patients right where they live. By combining in-person rounds with strategic telehealth coverage, you’ll stabilize residents during critical moments and prevent the trauma and disruption of unnecessary hospital transfers.
Key Responsibilities:
- Deliver comprehensive wound care treatment and management
- Provide medical care in LTC and skilled nursing facilities
- Collaborate with interdisciplinary teams to ensure continuity of care
- Maintain accurate and up-to-date patient documentation
- Conduct patient assessments, monitor progress, and adjust care plans as needed
Qualifications:
- Active Nurse Practitioner or Physician Assistant license in the state of Missouri
- Experience in wound care, geriatrics, or long-term care settings is preferred
- Strong clinical judgment and decision-making skills
- Excellent interpersonal and communication abilities
- Ability to work independently across multiple facility sites
Schedule:
- Full-time
- Monday to Friday with occasional flexibility based on facility needs

Pay: $125,000.00 – $130,000.00 per year
