Family Medicine PA-C / FNP – Primary CareFaith Medical Clinic
Job SummaryFaith Medical Clinic is seeking a motivated and dedicated Physician Assistant-Certified (PA-C) or Family Nurse Practitioner (FNP) to join our growing primary care practice in Westland, MI. Primary care/family medicine experience is preferred, along with proficiency in Epic EHR.

Schedule & Work-Life Balance
- Monday–Friday, 9:00 AM – 5:00 PM
- No weekends
- No on-call responsibilities
Key Responsibilities
- Provide comprehensive primary care, including preventive visits, annual physicals, and chronic disease management
- Evaluate and treat patients across all age groups
- Perform thorough assessments and document visits using Epic EHR
- Manage conditions such as hypertension, diabetes, obesity, and other common primary care diagnoses
- Educate patients on lifestyle, medications, and preventive care
- Collaborate with the care team to ensure high-quality outcomes
- Contribute to clinic growth, patient retention, and quality improvement initiatives
Qualifications
- Licensed PA-C or FNP (active and unrestricted)
- Primary care or family medicine experience preferred
- Epic EHR experience required
- Strong clinical and communication skills
- Ability to work independently and as part of a team
