Full Job Description

If so, consider joining the Regency Nursing & Rehabilitation Center team! We currently have a dynamic career opportunity available for a qualified MDS Coordinator. This is a remote hybrid position with MDS experience.

MDS Coordinator:

  • Applicant must possess strong communication, clinical, and organizational skills.
  • MDS experience preferred.
  • Current/active RN or LPN licensure required.

MDS Coordinator:

  • Transcribe and take appropriate action on physician’s orders as directed including but not limited to Care Plan changes.
  • Assist with performing all admitting and discharge assessments and complete all other necessary paperwork.
  • Help monitor all aspects of patient care.
  • Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff.
  • Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained.
  • Coordinate and prepare MDS assessments for all residents.
  • Oversee the transmission of MDS resident assessments.
  • Prepare, monitor, and update assessments, Care Area Assessments, and Care Plans, as necessary.
  • Participate in Care Plan meetings with residents and family members.
  • Be aware of residents’ exit to and reentry from hospitals.
