Position Summary
We are seeking an experienced Nurse Practitioner or Physicians Assistant with wound care experience to join our team as a 1099 independent contractor. This role involves evaluating and treating patients with chronic wounds in their homes and in skilled nursing or rehabilitation settings.
We provide appointments to help support your schedule, and there is also an opportunity for providers to develop their own referral relationships and generate additional appointments for increased income.

Responsibilities
- Evaluate and treat chronic, non-healing wounds
- Provide weekly wound treatment as clinically appropriate
- Complete accurate and timely documentation
- Maintain compliance with clinical and payer documentation requirements
- Travel locally to patient homes and partner facilities
Qualifications
- Active Nurse Practitioner or Physicians Assistant license in Kentucky and Ohio
- Required: prior wound care experience
- Medicare and Medicaid credentialing preferred, or willingness to enroll if needed
- Experience with house calls, mobile care, home health, geriatrics, or related settings is a plus
- Experience with E-Clinical (EMR) preferred (but we can train)
- Strong clinical judgment, independence, and professionalism
- Self-motivated with an entrepreneurial mindset

Compensation
- $200 per hour for time spent treating wounds
- 1099 independent contractor opportunity
