POSITION SUMMARY:
Advanced Practice Providers (Physician Assistants/Nurse Practitioners) will join an established team of providers delivering compassionate, professional, and ethical patient care in both the clinical office and hospital settings. Responsibilities include working collaboratively with a board-certified cardiologist, evaluation and management of cardiovascular conditions, formulation of diagnosis and assessments, as well as preparing thorough and accurate documentation of encounters and patient care plans.
ESSENTIAL JOB DUTIES:
- Complies with the individual governing board for NP/PA rules and regulations.
- Complies with all licensing, privileging, and credentialing guidelines.
- Performs health assessments including patient history and physical examinations.
- Supervises, orders, and evaluates diagnostic tests and incorporates them into patient care plans.
- Formulates appropriate diagnoses based upon history, physical examination, and clinical findings.
- Plans and provides care in collaboration with the patient and family within the framework of individual governing boards.
- Prescribes pharmacologic and non-pharmacologic therapies.
- Provides patient/family education related to health and lifestyle status.
- Evaluates the effectiveness of the plan of care and interventions by comparing patient responses to planned clinical outcomes.
- Coordinates and collaborates appropriately with all members of the health care team through timely, efficient, and thorough documentation.
- Participates in the education of health care providers and students through mentoring, precepting, providing in-service activities, and developing presentations.
- Stay abreast of current trends in healthcare practice and participate in continuing education to ensure optimal clinical knowledge and effective patient outcome skills.
- Develops clinical knowledge though continuing education programs and reviewing professional literature.
- Participates in interdisciplinary team meetings and appropriate facility meetings.
- Assists with emergent situations and calls.
- Participates in hospital rotations and call schedules.
- Other duties as assigned per the need of the organization and within scope of licensure.
