Family Practice Physician Assistant / Nurse Practitioner

Preferred Health and Wellness is seeking a dedicated, compassionate, and energetic Family Practice Physician Assistant (PA) or Nurse Practitioner (NP) to join our growing healthcare team.

The provider will deliver comprehensive, patient-centered primary care services to individuals across diverse populations and age groups. Responsibilities will include patient assessments, diagnosis, treatment, medication management, preventive care, health education, and management of acute and chronic medical conditions.

The PA/NP will work collaboratively with the Preferred Health and Wellness clinical team and other healthcare professionals to promote overall wellness, improve health outcomes, and ensure coordinated care. Essential Duties and Responsibilities

• Conduct comprehensive patient assessments, including medical histories, vital signs, physical examinations, and review of presenting concerns.
• Diagnose and manage acute and chronic medical conditions within the provider’s scope of practice.
• Develop individualized treatment plans based on clinical findings, diagnostic results, patient needs, and evidence-based standards of care.
• Prescribe medications and treatments in accordance with Georgia laws, professional standards, and Preferred Health and Wellness policies.
• Provide medication management, including monitoring treatment effectiveness, potential side effects, and patient adherence.
• Order, review, and interpret laboratory tests, diagnostic imaging, EKGs, and other appropriate diagnostic studies.
• Provide preventive healthcare services, wellness education, screenings, and health promotion interventions.
• Educate patients and families regarding diagnoses, medications, treatment options, lifestyle modifications, and expected outcomes.
• Perform appropriate clinical procedures within the provider’s education, training, certification, and legal scope of practice.
• Provide treatment for minor injuries, infections, acute illnesses, and exacerbations of chronic medical conditions.
• Identify changes in a patient’s condition and make appropriate referrals to specialists, emergency services, or other healthcare providers when indicated.
• Coordinate care with physicians, nurses, behavioral health professionals, case managers, specialists, and other members of the multidisciplinary team.
• Provide services through outpatient visits, telehealth, community-based care, and other settings as determined by the needs of Preferred Health and Wellness.
• Maintain accurate, timely, and complete medical records and clinical documentation in the organization’s electronic health record (EHR/EMR) system.
• Complete appropriate ICD-10, CPT, and other clinical documentation and coding requirements.
• Maintain compliance with HIPAA, OSHA, state and federal healthcare regulations, professional standards, and Preferred Health and Wellness policies.
• Participate in quality improvement initiatives, clinical meetings, care coordination activities, and organizational programs.
• Support continuity of care through appropriate follow-up, referrals, discharge planning, and coordination of post-acute services when applicable.
• Perform other clinical and administrative duties as assigned.

Qualifications

• Graduate of an accredited Physician Assistant or Nurse Practitioner program.
• Current, unrestricted license to practice as a PA or NP in the State of Georgia.
• Current national certification appropriate to the provider’s profession and specialty.
• Current Georgia prescriptive authority, as applicable.
• Current DEA registration, if required based on prescribing responsibilities.
• Experience in family practice, primary care, community health, outpatient medicine, or a related healthcare setting preferred.
• Strong knowledge of medical terminology, anatomy, physiology, pharmacology, and clinical assessment.
• Working knowledge of ICD-10 and CPT coding and appropriate clinical documentation practices.
• Demonstrated ability to evaluate, diagnose, and treat patients within the provider’s authorized scope of practice.
• Proficiency with electronic medical records/EHR systems.
• Strong communication, interpersonal, clinical reasoning, and patient education skills.
• Ability to work independently while functioning effectively as part of a multidisciplinary healthcare team.
• Commitment to patient safety, confidentiality, professionalism, and high-quality patient-centered care.

Preferred Qualifications

• Experience working with diverse patient populations.
• Experience with Medicaid and managed-care populations.
• Experience in community-based healthcare or integrated healthcare settings.
• Experience working with patients with both medical and behavioral health needs.
• Experience providing telehealth services.
• Experience with care coordination and referral management.
• Previous experience in urgent care, emergency medicine, hospital medicine, or other acute-care settings is a plus.
• Experience with outpatient procedures and minor office-based procedures is a plus.

Pay: $59.00 – $65.00 per hour
