Nurse Practitioner’s (Contracted) – Mobile Primary Care Athens/Commerce and Alpharetta Georgia
We are seeking a qualified and dedicated Nurse Practitioner to join our Atlanta mobile healthcare team providing primary care services to residents in Assisted Living and Memory Care communities throughout the Metro Atlanta area.
Requirements:
- Certified and licensed Nurse Practitioner through the Georgia Medical Composite Board
- Current AANP certification
- BLS certification (current)
- Experience in Family Practice and/or Geriatric Medicine preferred
- Strong computer and EMR documentation skills
- Reliable transportation
- Excellent bedside manner and professional communication skills
Position Details:
- Contracted, full-time position (4–5 days per week)
- No part-time positions available
- Primary service area will be Alpharetta, Athens and commerce with occasional coverage in surrounding areas
- We currently serve 60–70 Assisted Living and Memory Care communities
- Travel may include select locations such as Conyers, Loganville, Buckhead, Dunwoody, and surrounding Metro Atlanta cities
About the Role:
- Provide on-site primary care to residents in Assisted Living and Memory Care settings
- Collaborate with facility staff and interdisciplinary care teams
- Deliver compassionate, high-quality patient-centered care
- Maintain accurate and timely documentation using EMR
