Overview
Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly enhance patient care and well-being. Duties

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
  • Administer medications, injections, IV infusions, and other treatments following strict protocols and HIPAA regulations.
  • Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks to document patient information accurately and efficiently.
  • Assist with specialized procedures such as catheterization, tube feeding, venipuncture, sterile processing, and wound suturing while maintaining aseptic techniques.
  • Support patients with activities of daily living and complex needs such as dementia care, Alzheimer’s care, memory care, hospice care, and acute pain management.
  • Collaborate with multidisciplinary teams in settings like hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, outpatient clinics, and home & community care environments.
  • Conduct patient education on health coaching topics including medication management, nutrition, discharge planning, and infection control practices.

Requirements

  • Valid state licensure as a Licensed Practical Nurse (LPN).
  • Proven experience working with individuals with disabilities or in senior care environments such as long-term care or assisted living facilities.
  • Demonstrated proficiency with EMR/EHR systems like Epic or Cerner; familiarity with ICD-9/10 coding is preferred.
  • Experience in critical care settings such as ICU or emergency medicine is highly desirable; hospital or trauma center experience is a plus.
  • Knowledge of medical terminology and physiology to perform accurate patient assessments and diagnostic evaluations.
  • Ability to perform procedures including phlebotomy, IV insertion/infusion, sterile technique applications, and airway management.
  • Strong communication skills for effective triage and case management; ability to work with diverse populations including pediatrics (neonatal ventilator use), adults in acute or chronic conditions, and individuals with developmental disabilities.
  • Commitment to maintaining HIPAA compliance and infection control standards; physical ability to stand for extended periods and assist with mobility aids like Hoyer lifts when necessary. Pay: $100,000.00 – $125,000.00 per year
