Full Job Description
Post Acute Behavioral Health is seeking an experienced Psychiatric Mental Health Nurse Practitioner or Physician Assistant to provide in-person behavioral health care to seniors and medically complex patients in the Sacramento area (East to Northeast Sacramento).
This position offers competitive compensation with performance-based incentive opportunities and a diverse clinical caseload. You will treat patients experiencing complex, acute psychiatric needs while also providing preventive and long-term care to residents whose conditions are mostly stable but require thoughtful monitoring.
Our clinicians do more than complete visits. We continually sharpen our clinical skills, share knowledge with one another, and work as a team to deliver a new type of psychiatry that the post-acute setting has previously not see, resulting in quality scores increasing and buildings being more equipped to handle highly complex behavioral patients.
The Opportunity
Many patients in skilled nursing and senior living communities cannot safely travel to an outpatient psychiatric office or they have been receiving telehealth . We bring in-person high-quality psychiatric care directly to them.
In this role, you will:
- Conduct psychiatric evaluations and follow-up visits
- Diagnose and treat depression, anxiety, dementia-related behaviors, and other psychiatric conditions
- Manage psychiatric medications and develop individualized treatment plans
- Treat complex acute cases and provide preventive, long-term psychiatric care
- Coordinate with nursing teams, attending physicians, primary care providers, families, and facility leadership
- Participate in care planning, discharge planning, and interdisciplinary clinical discussions
- Monitor treatment response and adjust care plans as needed leadership, keeping them informed of all patients seen
- Educate patients, families, and facility staff
- Maintain documentation that supports clinical quality and regulatory compliance
What We’re Looking For- Required
- Active California license as a Nurse Practitioner or Physician Assistant
- DEA registration or ability to obtain one
- At least two years of psychiatric or behavioral health experience
- Strong clinical judgment, communication, and time-management skills
- Commitment to in-person, collaborative patient care
Preferred
- PMHNP certification or PA CAQ-Psych
- Experience in geriatrics, skilled nursing, post-acute care, or senior living
- Experience treating dementia-related behaviors and medically complex patients

Pay: $145,000.00 – $185,000.00 per year
